The receipt will include the required optometrist's provider number and the lens item number.

If your order includes lenses for two different people, and you both need a health fund receipt, please let us know when you have placed the order and we will write out the correct receipts and email them to you.

If you wish to ring your health fund and see how much rebate you will receive, the following common item numbers might be useful.
Disposable Contact Lenses (3 months supply) Item 852.
Disposable Contact Lenses (6 months supply) Item 852 x 2.
Disposable Contact Lenses (9 months supply) Item 852 x 3.
Disposable Contact Lenses (12 months supply) Item 852 x 4.
The 3, 6, 9 and 12 months supply is the length of time the lenses would last if worn every day.

When ordering if you have any notes to help us with your receipt you are welcome to write this in the 'comments' section of your order.  Eg this order is a 6 month supply for my health fund receipt etc.

Lost Receipts
If you lose your receipt and you need it  to make a health fund claim, contact us and we will email you a duplicate.

Note: NIB Health fund holders, as of October 2010, need to email a copy of their prescription each time they order if they wish to claim a rebate from NIB.
